Donation Instructions

Follow the instructions listed below. When completing the donation form, all fields are required in order to complete your donation. If you have any questions, please get in touch.

1. Download the donation form.
2. Print and complete the donation form (all fields are required).

4. Make your check payable to "New Horizons For Children's Fund at Pure Charity".

5. Mail your check and the completed donation form to:

6. Your donation will be complete once we receive and process your check. After your donation is processed, you will receive an email at the email address you provide on the donation form to confirm your donation and account.
